To unlock Horrific Visions, you must be level 50 (as of Dragonflight). Once you are level 50, you must unlock the Heart of Azeroth, Nazjatar, and the Heart Forge. If you have all this unlocked, you will receive the quest to unlock visions as soon as you enter the Chamber of the Heart in Silithus.
