No mask required for this pet to drop. The pet Eye of Chaos drops in the revisited Horrific Vision of Stormwind, from Old Town completion. The mob Eye of Chaos accompanies the end boss Mathias Shaw in that zone. Killing Mathias Shaw also means the mob Eye of Chaos appears in the final fight against Alleria Windrunner in the Cathedral. When collecting chests, there is a low chance for the pet Eye of Chaos to drop.
